Mutations are traits specific to Mutants selected on Character Creation. After you create your character, you can spend 4 Mutation points to gain a random new mutation. They are split up into 5 Categories as follows:

You start with 12 mutation points to spend at character creation. The costs vary between mutation and the more expensive ones are generally stronger than ones that cost less. Defects have negative point costs, allowing you to spend more points on desirable mutations.

Physical mutations can alter almost any attribute of your character's body, from number of limbs to attribute scores to Natural Armor. Most are passive, with a few granting activated abilities. Mental mutations, on the other hand, grant mostly activated abilities, with few passive bonuses.

A number of mutations cannot be gained by the player character at startup nor through random mutation, but appear in NPCs.
